HOME CARE PACKAGES

You can use funds from your Home Care Package to purchase a personal alarm.

Please discuss this with your provider. We can then send a quote for funding approval, then invoice your provider directly.

NDIS

Self-managed NDIS participants can access our service at CODE W Personal Alarms, if they:

- self manage the supports and services

- have someone else manage the supports and services  for them - a plan manager

- use a registered plan manager 

 

CODE W is not a Registered NDIS provider yet but our suppliers are , so we may be able to order through them for you if you can only use  a NDIS Registered provider. 

AGE FRIENDLY MELVILLE ASSISTANCE FUND  (AFMAF )

 

Financial assistance for seniors,over 60 yrs that  live in the City of Melville and require support, that is unavailable through other funds to assist independent living.

Melville residents are eligible for $200 per year - you can apply once per financial year 

The Seniors Assistance Fund ( AFMAF ) offers a choice of short-term or one-off support services to seniors living independently in the community within the City of Melville.

AFMAF was set up to provide financial assistance for seniors who live in the City of Melville and who require support that is unavailable through other funds such as Home Care Packages, to support them to live independently at home.

 

Eligibility:

The following eligibility criteria apply:

  • The fund will be administered to seniors experiencing financial hardship

  • One approved application per person per financial year

  • The applicant must be over 60 years of age.

  • The applicant must be a City of Melville resident.

  • All alternate avenues of funding must be considered prior to the application

  • The service must be essential to the wellbeing, rehabilitation, and safety of the recipient

Examples of Short Term Service:

  • Domestic assistance

  • Personal care

  • In-home and residential support

  • Social support including shopping assistance

  • Meal preparation

  • Continence aids

  • Transport

  • Counseling

 

Examples of One-Off Assistance: Personal Alarm

  • Personal alarms : items to help address safety concerns for example CODE W personal alarms

  • Occupational therapy including rails or other essential aids (referral from GP or Allied Health is required)

  • Home maintenance including gardening

  • Other items to help address safety concerns for example CODE W personal alarm pendants

  • Access to modifications to the home, for example, rails or ramps

 You will need a screenshot or scan of your Healthcare card

 Application takes approx 2 weeks to approve and you will be notified by email

 Send your paid invoice to AFMAF and they will deposit $200 into your bank account to reimburse you
